This is the" Information Age" and everything runs on information. Prior to an examination we ask you to fill out 2 forms. These can be opened here and be printed on your computer. Fill them out ahead of time at home where all your information is located. If you fill them here at the time of your examination you might not remember something or other. Bring them with you when you come in for your examination.
The other option is to fill the form here on-line and the information will be here when you arrive for your appointment.
