This patient is a 45 year old male.  This is first eye exam.  He is not having any vision problems and is here only because his wife insisted.  The white fluffy lesion in the circle is referred to as a cotton wool spot.  It is an area of infarct or dead tissue due to a vascular defect.   The most common causes of cotton wool are diabetes and hypertension.  His blood pressure measured 210/170.  He  could of had a stroke.

He is now on hypertension medication and his blood pressure is well controlled.
